This directory enables health practitioners working with culturally and linguistically diverse communities to easily find translated health forms and documents. This directory is intended to provide access to documents created and translated by New Hampshire health agencies and organizations.
The translated forms should be used in conjunction with their English
counterparts to ensure that all parties are aware of what is being communicated.
Each translated form also is posted in English.
All documents are intended for use by any healthcare organization and may be
distributed for not-for-profit purposes. These documents are posted, when
possible, in a Word format in order to allow for easy customizing. If your
organization wants to make other modifications, there are several
New Hampshire-based translation services which
may be helpful.
