Leading Change: Community Healthcare Outcomes Meeting Summaries and Presentations
The second meeting
of the Leading Change: Community Healthcare Outcomes Initiative was held
at the Foundation for Healthy Communities office on October 27, 2011.
The focus was on activity in the state targeting prevention of
readmissions for patients with Congestive Heart Failure.
Goal of this initiative is: Learn practical strategies to achieve
desired clinical outcomes and understand the long-term cost of treatment
by developing new integrated models of care through shared information
and education to improve health or maintain the quality of life for
patients with an acute illness or disease condition.
