Leading Change: Community Healthcare Outcomes
The Leading Change: Community Healthcare Outcomes project developed from an April 2010 health reform conference in Manchester, co-hosted by the NH Hospital Association and the Foundation, where Dr. Bruce Hamory from Geisinger Health System in PA was the keynote speaker. The goal of the Leading Change project is to learn practical strategies to achieve desired clinical outcomes and understand the long-term cost of treatment by developing new integrated models of care through shared information and education. Our focus is to improve health or maintain the quality of life for patients with an acute illness or disease condition. Congestive Heart Failure (CHF) was selected as the topic for initial focus through a survey of hospital leaders in 2010.
