NH Medication Bridge Program
Patient Information

General Patient Requirements for Participation in Patient Assistance Programs:

Although most pharmaceutical companies have different eligibility requirements there are some standard requirements:

1.      The patient must be a U.S. resident.

2.      The patient must not have any outside assistance in paying for their medications. This includes Medicaid, VA, private insurance, etc. If the patient has any prescription assistance he/she is not eligible for assistance from the pharmaceutical companies. The companies want to be the last in the line of defense.

3.      The monthly household income should be under $1,700.00 for a individual and under $2,300.00 for a family of two. 

4.      The patient must be taking a long-term medication.
