What Do We Do?
The Greater Nashua Healthy Community Collaborative, formed
in 1997, prioritizes its work using a variety of data
sources and input from community leaders.
Five criteria are applied in selecting issues to be
addressed by GNHCC:
• Burden of illness in the general and vulnerable population
• Availability of current data
• Capacity and willingness to collect focused outcome
information
• Local champions to lead a project
• Evidence-based best practice literature and tools
