Greater Nashua Healthy Community Collaborative Health care organizations, human service providers and public agencies serving the Greater Nashua area and working together to address health issues and coordinate effective community resources. Current Activities Working with local physician practices and community agencies on a collaborative effort to address childhood obesity. Convened the Nashua Tobacco-Free Coalition which works to heighten community awareness of the dangers of secondhand smoke and increase opportunities for tobacco cessation. Organized the
Nashua Walking Program, a joint effort of 8
organizations, which provides walking opportunities from
April – November. Past Accomplishments Researched transportation resources for healthcare appointments and published the “Transportation Resource Directory”. Created and distributed the “Lost Your Health Insurance? Here’s what you can do” brochure in English and Spanish to all primary care practices in the Greater Nashua area. Initiated the first citywide “Turn-Off-the-TV Week” for children and families in 2004 as part of our work on primary prevention of obesity. Organized and promoted a community-wide wellness event, “March into May” which focuses on increasing physical activity, and engaged more than 1,200 adults and children in its first year. The 21st Century After-School Program continues as active participants. Obtained baseline measures of cholesterol screenings from more than 15,000 patients in three targeted primary care practices Developed an interactive heart attack risk assessment
tool to be used in the primary prevention of cardiovascular
disease and provided educational sessions to 16 physician
practices.
